It is important to make sure that the transition to living at home goes as smoothly and as safe as possible.

We want to ensure you have a pleasant recovery experience regardless of the procedure from which you are recovering.  Our short-term care services will also allow the family caregiver or spouse to tend to their own care needs.

Before the discharge from the hospital, our team will seek the information needed from your doctor/social worker/discharge planner to ensure that you will get the best quality care there is for returning home.  We use this information along with client/family input to co-author a custom care plan designed for your unique needs including personal care, memory care, and even 24-hour care, if needed.  We understand how hard it can be to handle this process on your own, so having someone you can trust can make these challenging times much easier.

Benefits of Having Part Time Caregivers

After being discharged from the hospital, everything can seem a bit overwhelming. Surgery can also be very invasive on the body, which can disrupt the activities of daily living.  Physical trauma from surgery can cause many problems such as loss of function in the body. It is important after surgery to remember to take medications and maintain proper dietary requirements given to you by your doctor.  According to Johns Hopkins Medicine, infections can be or become noticeable within 30 days after surgery, and the chances of obtaining an infection from surgery is a 1%- 3% chance.  Should Physical Therapy or Occupational Therapy be required, our dedicated care team can work alongside therapists so that everyday life can become easier and less stressful.  Our caregivers can assist with all activities of daily living such as: meal preparation, transportation to doctor visits, medication reminders, prescription and grocery pick up, and most importantly following discharge orders from the doctor. With NOLA @ Home Care’s post-operative care services, we know that you will feel more comfortable being at home instead of in a facility which will help the road to recovery be much smoother.

The team at Nola @ Home Care will come out and visit with you before your procedure or even in the hospital, while still following COVID-19 guidelines.  We will co-author a care plan with the client and/or the family of the client to ensure the needs of post recovery care are being met.  We can work collectively with your hospital discharge to ensure a seamless transition of care from the hospital to your home. 

Our care professionals can also assist with transportation home post-procedure and to follow up medical appointments.  Many of our clients do not have family in the area and rely on our trusted team for transportation.  In the event specialized transportation services are required we work closely with several trusted medical transportation services that we can refer.

Our short-term care offers the same long-term care benefits many of our clients enjoy but in shorter duration.
