After we do our in-home assessment, we design a schedule to ensure seamless coverage. Our process does not allow for any gaps in coverage as we do not leave the client alone during shift change. Our shifts end only when the starting shift arrives to the home.
Care Assessment – During our initial in-home assessment, several members of our office staff will visit and review the details of your care. Each individual has unique needs, and we are committed to creating a person-centered care plan that aligns with your loved one’s specific goals and objectives. To ensure the best possible match, we also consider your elderly loved one’s personality traits when assigning caregivers.
Caregiver Selection Process – Once the assessment is complete, we initiate the process of selecting care professionals. Our office staff carefully reviews the client’s individual needs to identify suitable, trained care professionals from our roster. We then design a dedicated Care Team, typically comprising providers who can work multiple 12-hour shifts weekly. While our standard shift changes for 24-hour cases are at 7 AM and 7 PM, we offer flexibility as needed.
Constant Communication – Care plans are dynamic and evolve in response to the changing needs of the individual. Various factors, such as physician orders, prescription adjustments, cognitive decline, and mobility limitations, can necessitate modifications to the care plan. To ensure everyone is informed and aligned on the necessary actions, we maintain continuous communication with family members.
Absolutely. 24-hour assistance can be a temporary solution. In many cases, we are hired temporarily due to a recent hospital discharge or while a client is still undergoing rehabilitation.
Most medical health insurance plans do not cover in-home care costs. We are fully licensed through the Louisiana Department of Health so we do work with every major Long-Term Care (LTC) Insurance provider if your loved one does have an LTC policy.
Yes, with 24-hour care we establish a small team of care professionals to work in rotation of 12-hour shifts after we train them on the specific details of the individual care plan. Our goal is to create continuity in care by providing the same people working on day and evening shifts.
